Youth Application (required for all scouts) — only need to fill out one time, triplicate is not necessary.
Adult Application (only required for adult leaders — you will need to submit to a criminal background check)
Family Talent Survey (to determine the unique skills of each Scout Parent so we can best involve them in Pack activities)
Auto Insurance Form (required for any Scount Parent driving to an outing — only needs to be filled out once unless information changes)
BSA Medical Form (only need to fill out Part A and C (no medical exam required) unless you are going to an event longer than 5 days; needs to be filled out for Scout AND Parent; only needs to be filled out once unless information changes)
